Steps to enrolling at Toddler Time CDC are:

• Schedule a Tour by filling out the Contact Us here 

• Get your name on our Waitlist

• Pay your Registration Fee

• Complete Registration Packet to include:

  • Registration Form ​

  • Emergency Information Form

  • USDA Form

  • Shot Records

  • School Physical

 

All registration forms can be filled out online.

Please read through carefully and answer all questions to ensure a quick registration process.
